Job Summary
Diabetes Nurse – part‑time, 1 day per week or 2 sessions per week at Woodview Medical Centre. The role involves leading diabetes management and education for our practice population.
Responsibilities
- Collaborate with GPs and the wider clinical team to meet patient needs.
- Run nurse‑led diabetes clinics and support QOF achievement.
- Assess, plan, develop, implement and evaluate treatment programmes that optimise patient health in partnership with patients and carers.
- Deliver comprehensive diabetes care, including reviews and patient education.
- Maintain effective communication with patients and carers, acting as first point of call for diabetes‑related issues.
- Refer newly diagnosed patients to DESMOND.
- Request and interpret diagnostic investigations, informing patients of results.
- Discuss medication changes to achieve target HbA1c and lipid levels.
- Promote self‑management, lifestyle modification and preventative care.
- Proactively identify and manage treatment plans for patients at risk of complications associated with diabetes.
- Identify vulnerable groups, providing support, care and education to enable self‑caring.
- Monitor blood pressures and QRISK.
- Provide weight management advice and discuss and plan weight management programmes with patients, referring as necessary.
- Provide home‑based diabetic care where required.
- Work in close liaison with specialist tissue viability and podiatry services where necessary.
- Contribute to audits, service improvement and ongoing development of chronic disease management.
Person Specification
Essential
- Registered Nurse (NMC registered).
- Experience in diabetes management and patient education.
- Excellent communication and organisational skills.
- Commitment to evidence‑based practice and continued learning.
Desirable
- Primary care experience.
- Diabetes diploma or equivalent qualification (MSc desirable but not essential).
- Experience using EMIS/SystmOne.
Qualifications
Registered Nurse (NMC registered).
