Job description
Job Summary

Woodview Medical Centre are looking for an enthusiastic and motivated diabetes nurse to join our team and take a leading role in the management and education of patients with diabetes across our practice population. This position is offered on a part‑time basis for 2-3 sessions per week, depending on availability.

Main duties of the job
  • Work collaboratively with GPs and the wider clinical team to meet the needs of patients supporting the delivery of both drug and non‑drug based treatment plans.
  • Run nurse‑led diabetes clinics and support QOF achievement.
  • Assess, plan, develop, implement and evaluate treatment programmes that optimise the health and well being of patients in conjunction with the patient and/or their carers.
  • Deliver comprehensive diabetes care, including reviews and patient education.
  • Maintain effective communication with patients and/or carers to act as a first point of call for diabetes related issues.
  • Refer newly diagnosed patients to DESMOND.
  • Request and interpret diagnostic investigations and inform patients of results.
  • Discuss medication changes to achieve target HbA1c and lipid levels.
  • Promote self‑management, lifestyle modification, and preventative care.
  • Proactively identify and manage treatment plans for patients at risk of developing complications associated with diabetes and manage these patients.
  • Identify vulnerable groups and provide support, care and education to enable them to be self‑caring.
  • Monitor blood pressures and QRISK.
  • Provide weight management advice and discuss and plan weight management programmes with patients and refer as necessary.
  • To be able to provide diabetic care for patients in their own home if required would be desirable.
  • Work in close liaison with specialist tissue viability and podiatry services where necessary.
