Job description
Role Overview

Deputy Community Nurse Team Lead for Toressa Community Nursing Team in Torpoint and Saltash. Autonomous practitioner responsible for assessment and treatment of patients with complex health and social care needs. Manage clinical caseload within community nursing team with guidance from Community Matron, Team Leads and Integrated Care Team Leads.

Key Responsibilities
  • Provide holistic nursing assessment and high quality, safe, evidence-based care to patients and carers, aiming to maintain patients in their own homes.
  • Conduct physical examination, detailed history taking, diagnose deterioration in pre-existing conditions, and plan treatment.
  • Frequent travel to various locations throughout the county, including areas not accessible by public transportation, and manage travel requirements.
  • Focus on safe, effective admission avoidance and support timely discharge from hospitals and other care settings.
  • Implement personalised support plans and collaborate with the whole community resource to facilitate ongoing care management.
  • Work in partnership with patients, carers, and multidisciplinary team to offer support, advice, promote informed choice, maximise independence and optimise quality of life.
  • Provide clinical management and leadership to all staff within the team.
Key Qualifications
  • Deputy Community Nurse Team Lead qualification, or willingness to complete long term conditions qualification in post.
