Job description
Role Overview

Role is 21-35 hours depending on the right candidate.

Key Responsibilities
  • Providing flexible, coordinated wrap‑around support to enable efficient and effective navigation through health, social care and third‑sector services for people living with dementia (PLWD) from diagnosis through to end of life or admission into a care home.
  • Maintaining and effectively managing a caseload, providing wellbeing support plans that enable PLWD to adapt to changes and deliver suitable support based on their evolving needs.
  • Liaising with referring partners to receive, assess and process inbound referrals for the service.
  • Ensuring accurate client records are kept and all relevant information is recorded in line with GDPR.
  • Meeting contact SLAs: an initial contact within 48 working hours of referral receipt and face‑to‑face appointments arranged within 10 working days; wellbeing support plans supplied within 20 working days of the first appointment.
  • Ensuring that every PLWD and their carers/family members receive clear, timely information, advice and guidance about available support.
  • Working alongside individuals in a strengths‑based, person‑centred manner, enabling them to be in control and participate fully.
  • Championing the needs and rights of the individual, respecting choices, and supporting communication of their views; promoting rights‑based approaches throughout the Dementia Service.
  • Identifying additional ‘added value’ support opportunities via Age Cymru Dyfed’s other services (e.g., Information and Advice, Befriending Life Links, Digital Support, Practical Services) or external providers.
  • Providing structured, ongoing support and follow‑up reviews or keeping‑in‑touch calls tailored to PLWD’s needs.
  • Facilitating service users’ ability to communicate in their language of choice.
  • Developing networks with relevant external professionals and partner organisations working in dementia, memory loss, and related health and social care fields.
  • Raising community awareness to support West Wales becoming a dementia‑friendly region.
Essential Skills
  • Ability to listen and communicate effectively, establishing rapport with people.
  • Knowledge of the complex issues faced by people living with dementia and their carers in West Wales.
  • Understanding of mental health and well‑being issues, including dementia and carer stress.
  • Awareness of primary legislation in Wales that addresses these issues.
  • Broad understanding of health and social care issues for people living with dementia and their supporters.
  • Experience adhering to policies and procedures, including consent, GDPR, adult safeguarding, etc.
  • Objective, person‑centred work style, effectively liaising with clients, carers, health and social care professionals, and other service providers.
  • Capacity to work autonomously; manage time effectively and prioritise referrals targeted at those in greatest need.
  • Commitment to collaborative working, co‑production, and ability to make home visits as required.
Desirable
  • Knowledge of the welfare benefits framework and ability to undertake benefits checks and provide advice up to level 4 of the advice framework (training provided).
  • Knowledge of the framework and organisation of service‑providing agencies relevant to older people living with dementia within Dyfed.
  • Spoken and written Welsh to a minimum of level 3.
  • Previous experience working with individuals living with dementia and/or their carers.
