Job summary

Avery Healthcare is seeking a warm, enthusiastic Dementia Care Manager for Roseberry Manor Care Home in Epsom. You will lead the dementia community, implement strategies, and manage resources within budget while ensuring person-centred care plans and strong family communication.

The role requires at least three years in a similar setting, knowledge of CQC standards, and UK health and social care market understanding. DBS disclosure will be required and costs covered by Avery.

Responsibilities

• Oversee the Dementia Community's overall management and implement the dementia care strategy within budget.
• Ensure the care plan day is accurate and reflects residents' needs and preferences.
• Create and maintain person-centred care plans with integrated information.
• Build caring relationships with residents and families, and communicate sensitively.
• Act as advocate for residents' best interests and manage conflicts appropriately.
• Collaborate with Activities Team to develop meaningful activities for residents with dementia.
• Promote a culture that maximises residents' independence and quality of life.

ABOUT THE ROLE

• Your focus as a Dementia Care Manager will be tobe responsible for the overall management of the Dementia Community, implementing and embedding thecompanys dementia care strategy and managing resources within a given budget.
• Ensuring that the plan care day is accurate and reflected in the care needs summary and that the resident/relative identifies needs and preferences.
• Ensure all information is integrated into a person-centred care plan.
• Building caring relationships with residents and families, liaising with them sensitively and appropriately.
• Act as an advocate for the residents' best interests, appropriately managing conflict.
• Working closely with the Activities Team to ensure the development of meaningful activities for residents with Dementia.
• Support and set expectations for the care team's participation in these activities.
• Promoting a culture that enables residents to maximise their capabilities, optimising their independence and enhancing their quality of life.

ABOUT YOU

• A minimum of three years experience in a similar role, working within a fast-paced environment.
• Comprehensive understanding of CQC standards and requirements.

• Knowledge of care services businesses and the UK health and social care market.
