Job summary

Rotherham Doncaster and South Humber NHS Foundation Trust is seeking a Clinical Dementia Advisor to join a multidisciplinary CMHT. The post focuses on review and coordination of care for patients with dementia who have already been diagnosed, operating within a tiered care model in partnership with the Alzheimer's Society led by Rdash.

You will lead clinical assessments, plan and evaluate evidence-based care, advise partner agencies, and mentor junior staff.

Qualifications

• Significant experience of working with people with a diagnosis of dementia.
• Understanding of the needs of people affected by Dementia.
• Working in the community as part of an MDT.
• Carer support and education.

Responsibilities

• Be responsible for the further assessment, planning, implementation and evaluation of the evidence-based care required for identified dementia patients.
• Coordinate reviewed care plans within MDT support and ensure health promotion.
• Provide professional advice and support to other agencies and team members.
