  • Answer medicine-related enquiries from GPs, practice staff, and patients.
  • Review and reconcile medicines post-hospital discharge.
  • Set up and manage systems for continuity of medicines supply.
  • Contribute pharmaceutical advice for new services.
  • Analyze and interpret medicines data for decision-making.
  • Undertake audits of prescribing and contribute to research initiatives.
  • Implement changes to medicines based on guidance.
  • Monitor prescribing against local health economy’s RAG list.
  • Assist practices in maintaining a practice formulary.

Responsibilities
  1. Conduct medication reviews for patients with single or multiple medical problems, ensuring medicines optimization.
  2. Make appropriate recommendations to Senior Pharmacists or GPs for medicine improvement.
  3. Attend and refer patients to multidisciplinary case conferences, managing caseloads for common/minor ailments.
  4. Provide patient-facing clinics and a telephone help line for medication-related queries.
  5. Answer all medicine-related enquiries from GPs, practice staff, healthcare teams, and patients, suggesting and recommending solutions.
  6. Review and reconcile medicines post-hospital discharge and upon admission to care homes.
  7. Set up and manage systems to ensure continuity of medicines supply to high-risk patient groups.
  8. Contribute pharmaceutical advice for the development and implementation of new services with medicinal components.
  9. Analyze and interpret medicines data, presenting findings to support decision-making.
  10. Undertake simple audits of prescribing and contribute to national and local research initiatives.
  11. Implement changes to medicines based on MHRA alerts, product withdrawals, and other guidance.
  12. Monitor practice prescribing against local health economy’s RAG list and make recommendations to GPs.
  13. Assist practices in maintaining a practice formulary and auditing compliance against NICE technology assessment guidance.
  14. Provide newsletters or bulletins on important prescribing messages.
  15. Deliver education and training to primary healthcare teams on therapeutics and medicines optimization.
  16. Ensure compliance with CQC standards where medicines are involved.
  17. Support public health campaigns and provide specialist knowledge on all public health programs.
