Critical Care Outreach Practitioner

An opportunity hasarisen for a member of Critical Care staff to jointhe Critical Care Outreach Team at South Tyneside and Sunderland FoundationTrust.

You will deliver a nurse-led critical care outreach service, to support and improveservices for level one and level two patients on the Ward, who are at risk ofdeteriorating.

You will providehighly-specialised interventions and support with appropriate physicalassessment, observation and treatment of acutely / critically ill patients. You will contribute to the developments of the CCOT service and educationthroughout Trust when required.

KeyRequirements: Critical Care nurse with significant Band 6critical care experience.

Hoursper week: 37.5 hours per week, but would consider part-time. Must be able to work day and night duty, including public holidays and weekends.

Main duties of the job

You will be required to provide high-quality nursing care to patients atward level and within critical care throughout the Trust. You will be requiredto support and improve services for patients at risk of deterioration, andthose who may need escalation to a higher level of care. You willbe required to provide highly-specialised interventions and support with theappropriate physical assessment, observation and treatment for acutely-unwellpatients, and support those stepping down from a higher level of care. You will need to have significant critical care experience at a seniorlevel.

The CCOT service provides 24/7 cover to bothSunderland and South Tyneside sites, so rotation is required. The Post also requires you to be able to work a range of shifts including nightduties, weekends and bank holidays.

Experience
  • Experience of care delivery.
  • Significant Band 6 experience of working in Critical Care.
  • Experience of leading a team/supervising junior staff.
  • Evidence of innovation in practice.
  • Evidence of practice development.
  • Experience of co-ordinating care for a caseload of patients.
  • Clinical teaching experience.
  • Experience of working within the Critical Care Outreach Team.
Skills & Knowledge
  • Awareness of national Health and Social Care policy.
  • Knowledge of best practice in area of specialism.
  • Thorough understanding of expanded roles/professional practice issues.
  • Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required.
  • Able to work without supervision.
  • Competent in the use of IT packages (Meditech, V6, MS Office).
  • Evidence of competency in relation to clinical practices appropriate to the Post.
  • ALS course.
Qualifications
  • Registered Nurse.
  • Degree in Nursing studies or other relevant clinical Degree (UK
  • equivalent).
  • Mentorship Certificate / Practice Assessor training or demonstrable equivalent experience.
  • History Taking / Clinical skills accreditation.
  • Completion of National Competency Framework for Registered Nurses in Adult Critical Care Steps 1, 2 & 3, or equivalent.
  • Post-Qualification Critical Care course which meets the requirements of the National Standards for Adult Critical Care Nurse Education.
  • Leadership and Management course.
  • Non-medical prescribing or willingness to complete.
