Cheshire and Wirral Partnership (CWP) invites applications for a full-time Mental Health Practitioner based in our First Response Services within the Crisis Resolution Home Treatment Team, Chester. You will work across Urgent Care Mental Health First Response Services, performing comprehensive crisis assessments and delivering evidence-based interventions.

Qualificações

• Recognised qualification in mental health - RMN, AHP, Social Care (AMHPs).
• Evidence of further professional development involving serious mental illness.
• Willingness to undertake nurse prescribing certificate.
• Non-medical Prescribing certificate; information management and technology skills.

Responsabilidades

• Work within a multi-disciplinary team, providing comprehensive crisis assessments and interventions.
• Undertake detailed assessments of people in crisis and develop care plans.
