Job description
Overview

37.5 hours per week. Doncaster Crisis Team: a role involving triage assessment and intervention for service users in crisis, including telephone contact, home visits and assessments in A&E. A permanent Band 6 qualified practitioner is sought to join a busy team, with a focus on delivering a caring and safe service in the community.

We are looking for dynamic, positive practitioners who work within a strengths and recovery based approach and who are passionate about supporting individuals in crisis.

Responsibilities
  • Provide triage assessment and intervention for service users in crisis.
  • Conduct telephone contacts, home visits and assessments in A&E as part of crisis management.
  • Liaise with families to support their understanding of crisis management.
  • Work with partner organisations, third sector and peer support services to deliver coordinated care.
  • Contribute to and lead changes as part of the community working age model and crisis service development.
Qualifications and Requirements
  • Permanent Band 6 qualified practitioner.
  • Enthusiastic, highly motivated and reliable to work in a busy crisis service.
  • Passion and desire to work with individuals in crisis within the community, delivering a caring and safe service.
  • Progressive, strengths- and recovery-based approach.
  • Ability to liaise with families and partner organisations in a transparent and open manner.
