Crisis and Home Treatment Practitioner

We are looking to recruit a Band 6 qualified RMN into our Home Treatment team. We are looking for someone who is enthusiastic and highly motivated to join our busy team and has a passion and desire to work with individuals and families within the community delivering a caring and safe service.

You will be joining a supportive and passionate team, who will support you through an induction, so that you can familiarise yourself with local provisions and services.

The service is an alternative to hospital admission, as well as facilitating early discharge from the Acute inpatient wards.

The post will be within the Acute Directorate and the successful person will be required to demonstrate their confidence as a mental health practitioner with excellent and robust assessment and risk management skills and can manage complex situations and work well under pressure.

The post will involve flexible duty rota covering Home Treatment seven days a week 9am-7pm but also Crisis Teams shifts which are operational throughout a 24-hour period including night shifts.

Main duties of the job

This is an exciting time to work in Rotherham Urgent care services. We comprise of Crisis, Home Treatment and Hospital Liaison. We have strong links with the inpatient units, and the Locality teams. We also work closely with Partner agencies and third sector organisations to ensure we provide a comprehensive service to our community.

There are exciting developments within the Home Treatment team, as we work towards accreditation.

We are looking for passionate, reliable and progressive staff who work within a strengths and recovery-based approach.

Liaison with families and supporting their understanding of crisis management is an important part of the role.

The post will require individuals to work alongside partner organisations, third sector and peer support services, transparency and openness is vital.

You must have a qualification in Mental Health Nursing, Social Worker or Occupational Therapist. You must be registered with relevant governing bodies.

  • Evidence of post registration training
Experience
  • Demonstrable post registration experience as an RMN
  • Professional experience of working in acute adult mental health.
  • Experience of MDT working
  • Skills in coordinating and planning inter‑agency approaches in mental health care.
  • Experience of assessing and managing risk
  • ENB 998 or other NMC approved mentor qualification or mentor qualification appropriate to discipline‑or willingness to complete.
Knowledge
  • Ability to use initiative and work without immediate supervision
