The Forward Trust is seeking a Continuity of Care Coordinator to work primarily at HMP Brixton, with travel to London sites as needed. You will lead audits, coordinate pre-release and discharge processes, and strengthen links between prison teams and community substance misuse services.

We're looking for an enthusiastic and organised Continuity of Care Coordinator to join our substance misuse team at HMP Brixton. This is an exciting opportunity to play a key role in strengthening the journey from prison into community treatment, building effective partnerships and helping ensure people leaving custody experience a safe and supported transition.
Based primarily at HMP Brixton, you'll work across our London services, including HMP Wormwood Scrubs and Heathrow IRC, so some travel will be required. Reasonable travel expenses will be covered.
