Job description

Youll work alongside our Band 7 Continence Specialist Nurse as part of a close-knit, community-based continence team delivering high-quality care across Swale.

Your role will involve a blend of clinical, office and community work, giving you the opportunity to build strong patient relationships, support complex care planning, and contribute to service improvement. You may also work with other local teams to provide occasional cross-cover where required, helping ensure continuity of care across the region.

This is a full-time position, offering stability, variety, and ample scope for continued learning and professional development.

Main duties of the job
  • Carry out comprehensive continence assessments, treatment planning and follow‑up care
  • Work collaboratively with community nursing teams, acute hospitals, AHPs and social care colleagues to deliver a coordinated, high-quality service
  • Manage and advise on complex continence cases, supporting the wider MDT to ensure seamless care pathways
  • Promote best practice in continence management, adhering to local and national guidelines
  • Support patient education on bladder and bowel health, empowering individuals to manage their conditions confidently
  • Maintain accurate, timely clinical records and contribute to audit, service development and quality improvement initiatives
  • Provide professional advice and support to colleagues, carers and patients to ensure safe, effective care
  • Participate in ongoing training, supervision and reflective practice to enhance your clinical expertise
