Avon and Wiltshire Mental Health Partnership NHS Trust is seeking a Consultant to provide medical input to an older adult population across West Wiltshire, including dementia services and memory care. Responsibilities

Provide advice to members of the Multi-Disciplinary Teams in West Wiltshire and the North Wiltshire Dementia Service.
Regular remote attendance at MDT meetings.
Assessment and treatment of outpatients in virtual and clinically present clinics.
Hold CPA meetings for outpatients.
Visit patients in their own residence and/or care homes if clinically indicated.
Provide medical leadership into the Dementia Service including Care Home Liaison.
Liaise with other parts of the service, GPs and non-NHS organisations as applicable.
Liaise with Intensive and Inpatient services to facilitate handover of clients.
Prepare Mental Health Act Tribunal reports and participate in related meetings.
Provide medical and AC responsibility for patients from designated GP practices.
Participate in team and personal caseload management and supervision.
