The post is a replacement 10 PA post. Sedgefield treatment and intervention team is based in Spennymoor. The team provides secondary mental health service input for all mental health conditions for working age adult population. The team receives referrals mostly from GP surgeries via central Access team and other sources such as Crisis team / Liaison Psychiatry / wards. The team provides specialist support to patients with significant mental health difficulties such as mood disorders, schizophrenia, bipolar disorder, personality disorder and adult neurodevelopmental conditions such as ADHD and Autism. The team is able to obtain expert advice from Trust-wide specialist services which include an ADHD service, the Autism Spectrum Assessment Team and the eating disorder service. The Crisis Team manages access to inpatient services. The post-holder will develop effective working relationships with members of the Crisis Team and Inpatient Team by closely liaising with these services when patients are referred to the crisis team or needing alternative to hospital beds. Patients who need to be in hospital will be managed by the inpatient team. The post holder will join a dedicated multidisciplinary team and contribute to compassionate and evidence-based patient care and service improvement.
Situated within Spennymoor, County Durham, working into Sedgefield North PCN which consists of 10 GP Practice groups in the area.
The Consultant will work as part of a multi-disciplinary team and provide clinical expertise for both new and ongoing cases, particularly issues around psychopharmacology, diagnosis, and risk assessments including liaison with the local GPs and inpatient units. All referrals for inpatients will be seen initially by the Crisis Team who are based nearby. West Park Hospital is the receiving mental health inpatient unit. All referrals for inpatients will be seen initially by the Crisis Team who are based at West Park Hospital, Darlington.
Most outpatient work will be at Enterprise House with some clinics based at the Pioneering Care Centre in Newton Aycliffe. There will also be an expectation to do home visits and outreach as necessary. The consultant is expected to liaise with inpatient teams for instance formulation meetings and transfer of care meetings.
