  • Assess and treat outpatients in clinics.
  • Hold CPA meetings for outpatients.
Job description
Key Responsibilities
  • Deliver direct interventions to service users at a level of intensity according to need, including those requiring an assertive approach and those who are difficult to engage.
  • Conduct ongoing assessment, diagnoses (including review and clarification) and treatment of mental and physical health needs.
  • Provide medical input and leadership to the WWKD CMHT, with particular responsibility for patients from surgeries in Melksham and Trowbridge.
  • Provide advice to members of the Multi-Disciplinary Teams in the WWKD CMHT team and attend regular MDT meetings.
  • Assess and treat outpatients in clinics.
  • Hold CPA meetings for outpatients.
  • Visit patients in their own place of residence if clinically indicated.
  • Liaise with other parts of the service, GPs and non-NHS organisations wherever applicable.
  • Liaise with the Intensive and Inpatient services as appropriate to facilitate the smooth handover of clients between services.
  • Fulfil the Responsible Clinician role with respect to patients subject to Community Treatment Orders.
