Role and Team

This is a replacement post. In 2021, the Trust was successful with our bid to implement our Specialist Community Forensic Team (SCFT) model across the whole of North West and Central London. The service covers 8 North West London boroughs in three hubs (Brent, Harrow & Hillingdon (BHH), Ealing, Hammersmith & Hounslow (EHH) and Kensington, Chelsea & Westminster KCW).

Responsibilities and Expectations
  • Work within a dynamic and progressive SCFT team with a small but complex caseload of approximately 30 outpatients, supported by a well-resourced multidisciplinary team including social care and administration.
  • Provide in-reach to secure wards and follow up to 30 in-patients prior to discharge, but you will not be the Responsible Clinician (RC) for these patients.
  • Offer clinical leadership to the team and act as Responsible Clinician / Clinical Supervisor for the service users in your caseload, who will also have an assigned social supervisor and community psychiatric nurse (CPN).
  • Offer community forensic expertise, advise and support to secure ward teams and service users preparing for discharge.
  • Provide opportunities to apply for funded sessions or use SPA time for leadership, medical education, research, QI and other linked activities.
  • Support professional career development and co-create the job with your interests where possible.
