Job description
Overview

Stockport Foundation Trust are looking for a dynamic, innovative Advanced Practioner with a wealth of experience to join the team. This will be a lead role as Consultant ACP that will continue to drive the Divisional agenda for Advanced practice. The post will be structured around four core functions that exemplify the role: o An expert clinician / practice function o A professional leadership and consultancy function o An education training and development function o A research, evaluation and service development function. To be responsible for developing, implementing and reviewing the Advanced Practice ensuring that the service is delivered equitably and to a consistently high standard. The post holder will be responsible for leading, developing and implementing innovative and creative models of care management for patients across community and assist in the implementation of urgent treatment centre models. The post holder will utilise their skills in clinical examination, assessment and planning care for patients. They will provide expert professional advice to other professionals, multidisciplinary groups, patients and their carer, as well as contributing to the operational and strategic developments to provide a more co-coordinated and equitable approach to advanced nursing across community services and will be responsible for maintaining and enhancing professional standards of care., The consultant ACP will be responsible for leading, developing and implementing innovative and creative models of care management for patients within the community setting and assist in the implementation of and development of future models. The post holder will utilise their skills in clinical examination, assessment and planning care for our patients. They will provide expert professional advice to other professionals, multidisciplinary groups, patients and their carers, as well as contributing to the operational and strategic developments to provide a more co-coordinated and equitable approach to the specialty and will be responsible for maintaining and enhancing professional standards of care. With the aim of improving the quality and efficiency of patient focused systems and processes the post holder will have responsibility of develop a robust strategy plan to modernise the ACP team and to provide a high quality, patient-centred service for patients. The post holder will also provide expert clinical and professional advice to staff within Community and across areas of the Trust. The post holder will deliver a 65% clinical component to their role and 35% related to leading clinical practice, research and innovation, teaching and training.
