Role Overview

We are seeking a committed and organised Complex Needs Support Officer to join our office-based team. This role involves working directly with individuals to assess needs, coordinate support, and maintain accurate records, ensuring that service users receive holistic, timely, and effective assistance.

Key Responsibilities
  • Conduct assessments and develop person-centred support plans for adults with complex needs.
  • Provide guidance and support to service users, including welfare, housing, and community resources.
  • Monitor progress and liaise with internal teams and external agencies to ensure coordinated care.
  • Maintain detailed and accurate records in line with data protection and organisational policies.
  • Support safeguarding and risk management processes.
  • Attend team meetings, case reviews, and contribute to service improvement initiatives.
