Job summary

The London North West University Healthcare NHS Trust is recruiting a Clinical Nurse Specialist for complex colorectal cancer at one of its central London sites. The post holder will join a dedicated multidisciplinary team delivering tertiary care for locally-advanced and recurrent rectal cancers, coordinating patient pathways with leadership, innovation and clinical expertise.

The role includes leading care planning, facilitating pre- and post-operative nursing care, and coordinating clinics

Qualifications

• Registered Nurse Degree level
• NMC registration
• Evidence of continued professional development relevant to clinical area
• Recognised teaching qualification or working towards
• Masters degree or working towards
• Fundamentals in Cancer Care (Level 7 module) or equivalent

Responsibilities

• Implement processes along the patient pathway to ensure it is managed efficiently, smoothly and in accordance with targets and agreed timescales.
• Work with clinical areas to facilitate a co-ordinated and streamlined pathway of care which revolves around the patient and their family’s individual needs.
• Provide robust timely and comprehensive assessment and personalised care planning.
• Support the delivery of pre- and post-operative nursing care and personalised discharge planning.
• Co-ordinate and deliver comprehensive arrangements for communicating with patients and their local teams (who may be located far away).
• Support facilitation of clinics and multidisciplinary team meetings to meet the standards and requirements of specialist commissioning and the national framework.
