Job summary

Swanton Care & Community Ltd Derbyshire is recruiting a Support Worker to join a core community team. You will provide practical and emotional support to people living independently, following individual support plans and safeguarding principles.

The role requires flexibility across day, night and weekend shifts, full-time 37.5 hours weekly, with a focus on dignity, empowerment, and inclusive, person-centered care.

Responsibilities

• Provide practical assistance and emotional support to empower people to achieve their goals, respecting individuality and enabling informed choices.
• Promote health, well-being, and inclusivity in line with cultural and spiritual beliefs.
• Facilitate social, educational or work activities such as visits with family, walks, shopping and other chosen activities.
• Perform general housekeeping and personal care tasks as needed.
• Complete a range of records and documents to maintain accurate care information.
