Job summary

Outward is seeking proactive, compassionate Support Workers to join our teams in Tower Hamlets and Hackney, London. You will provide direct, person-centred support to adults with learning disabilities, autism and mental health needs, helping them live as independently and safely as possible.

Responsibilities include daily living support, community access, health appointments, safeguarding and building positive relationships with families, colleagues and professionals.

Qualifications

• Good safeguarding knowledge and risk management in care/support settings.
• Confidentiality, GDPR and data protection principles.
• Smartphone access for secure system login (SMS/auth apps).
• Right to Work in the UK; visa sponsorship not provided.

Responsibilities

• Provide direct, person-centred support to adults with learning disabilities, autism, mental health needs and/or complex support requirements.
• Help develop daily living skills, independence, community access and personal goals.
• Assist with medication support, health appointments and day-to-day tasks per support plans.
• Act as a key worker, contribute to plans, risk assessments and records.
• Promote safeguarding, inclusion, dignity and positive risk-taking.
