Job summary

Leaf Complex Care Group is seeking a Healthcare Assistant to provide compassionate, person-centered care primarily in clients' homes within the community. You will support individuals to live independently and with dignity, tailoring care to diverse needs and coordinating with families and care teams.

The role offers flexible shifts (4–8 hours or longer) with varying patterns based on care packages, and opportunities for specialist training and career development within a supportive team.

Qualifications

• NVQ Level 3 in Health & Social Care (Level 2 acceptable).
• Minimum 1 year of experience in similar healthcare settings, with complex needs, behaviours that challenge, and autism.
• Full UK driving license would be advantageous.

Responsibilities

• Deliver high-quality, person-centered care in individuals' homes and communities.
• Support individuals in developing skills and personal interests.
• Adapt care plans to meet diverse needs, ensuring comfort and safety.
• Communicate proactively with families and care teams for effective delivery.
• Support individuals in residential placements or their own homes.
• Perform household tasks such as laundry, cleaning and shopping.
• Budget for community activities and manage finances with support.
• Assist with personal care including hygiene, dressing and feeding.
• Maintain records and communicate with the brand team and healthcare professionals.
