Job summary

Your Choice Your Future is seeking caring Community Support Workers to assist children and adults with disabilities in their local communities. This field-based role requires person-centred support, promoting independence and social participation.

You will help individuals access community activities, attend appointments, and follow care plans, while maintaining safeguarding and company policies. No prior experience is essential if you have the right attitude and willingness to learn.

Qualifications

• Experience with disabilities is beneficial but not required.
• Ability to work in community settings and support individuals locally.
• Commitment to person-centred care and safeguarding guidelines.

Responsibilities

• Provide friendly, respectful and person-centred support to children and adults with disabilities.
• Support individuals to access community activities, appointments, leisure activities and social opportunities.
• Encourage independence, confidence and life skills wherever possible.
• Follow individual care and support plans and ensure their needs are met safely.
• Promote choice, dignity, inclusion and independence at all times.
• Communicate effectively with individuals, families and colleagues.
• Complete relevant daily records and report any concerns appropriately.
• Follow safeguarding, health and safety and company policies and procedures.
