Job summary

A healthcare organization is seeking a Community Support Worker for the Intensive Home Treatment team in Stapleford. This role involves providing support to clients aged 65 and over with severe mental health issues, promoting recovery, and working alongside a multidisciplinary team. Applicants must have excellent communication skills and the ability to manage complex cases. Note that this position does not qualify for Skilled Worker visa sponsorship.

Qualifications

  • A valid UK driving licence is required for travel.

  • Deliver intense support within clients' homes to prevent hospital admissions.
  • Assist in assessments and interventions as delegated by other professionals.

Job description

This is an exciting opportunity to be part of the Intensive Home Treatment team as a Community Support Worker within the Broxtowe area if you are seeking to develop your skills and expertise in working with clients aged 65 years and over living in the local community and experiencing severe mental health problems.

You will be an integral member of a dynamic multidisciplinary team offering creative and consistently high quality evidence‑based interventions.

Please note that this post does not meet the pay or skill level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa, and the Trust would not, therefore, be able to sponsor for a Skilled Worker/Health & Care visa.

Responsibilities
  • Deliver intense input within the person's home and assist clients to avoid hospital admission.
  • Contribute to assessments and interventions, undertaking nursing, therapy and social activities as delegated by the relevant profession.
  • Frequently work without direct supervision in the community, with flexibility and a holistic approach to meet this client group’s needs.
  • Promote recovery of patients and work with partner agencies and primary care to ensure timely and safe discharge from secondary care.
Qualifications & Experience
  • Excellent communication and interpersonal skills.
  • Experience managing complex cases in partnership with other agencies.
  • Full UK driving licence and vehicle for business use (reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010).
  • Motivation to work as an enthusiastic team player within a dynamic multi‑disciplinary health team.
