Job Summary

We have a rare and exciting opportunity for a suitable experienced and enthusiastic Therapist who has appropriate experience and knowledge of post‑stroke care, ideally in a community setting. Our multidisciplinary community stroke team provides rehabilitation support to patients in their own homes and bed‑bases across the footprint of CCICP, utilising a Home First approach and proportionate care while working closely with acute and community colleagues.

As Team Leader you will be based with the Community Stroke Team at Infinity House, acting as a visible and present leader who is professionally responsible and accountable for the delivery of quality, safe patient care by therapists within your service. You will oversee staff development, education, shadowing and supervision and work closely with the Community Operational Manager to maintain and develop the service.

Key Responsibilities

• Lead, manage and coordinate the Community Stroke Service, implementing and monitoring Trust policy, protocols and patient pathways.
• Oversee staff development, education, shadowing and supervision of therapists.
• Work closely with the Community Operational Manager to effectively manage budget and service development.
• Collaborate with referring hospitals and other community services to ensure timely care and rehabilitation for post‑stroke patients.
• Demonstrate positive leadership skills, identifying and prioritising workload and driving clinical excellence and service development.

Qualifications

• NMC registered candidate or AHP.
• Experience of continuous improvement processes is advantageous.
• Suitable experience and enthusiasm for post‑stroke care in a community setting.
• Ability to lead and manage a multidisciplinary team.
