A community healthcare provider seeks a Therapist to lead the Community Stroke Service in England. You will be accountable for the delivery of quality patient care and the management of the team, implementing best practices in post-stroke care. The ideal candidate will have experience in community settings and the ability to drive clinical excellence. This role offers an opportunity to significantly impact patient rehabilitation while collaborating with multidisciplinary teams and operational managers.
We have a rare and exciting opportunity for a suitable experienced and enthusiastic Therapist who has appropriate experience and knowledge of post‑stroke care, ideally in a community setting. Our multidisciplinary community stroke team provides rehabilitation support to patients in their own homes and bed‑bases across the footprint of CCICP, utilising a Home First approach and proportionate care while working closely with acute and community colleagues.
As Team Leader you will be based with the Community Stroke Team at Infinity House, acting as a visible and present leader who is professionally responsible and accountable for the delivery of quality, safe patient care by therapists within your service. You will oversee staff development, education, shadowing and supervision and work closely with the Community Operational Manager to maintain and develop the service.
