Job summary

Hollister Incorporated is seeking a Stoma Care Specialist or a Nurse/District Nurse to join our community stoma care service covering Wakefield and surrounding areas. You will deliver first-class specialist nursing care in patients' homes, supporting the pathway from hospital to community.

This permanent role is Monday to Friday, 37.5 hours, with a band 7 equivalent. Driving licence is essential for home visits, and the team values education, patient empowerment and collaboration with GPs,

Qualifications

Community / district nurse experience is desirable.
Recognised Stoma Care Qualification is desirable.
Driving Licence is essential for home visits.

Responsibilities

Develop and deliver a community-based stoma care service in patients' own homes as part of a secured partnership.
Provide safe, effective and specialist nursing care, information and advice to patients and their relatives post stoma surgery.
Provide teaching and support to patients on how to independently care for their stoma.
Maintain good communication links between acute teams, GPs, Community Nurses and other disciplines.
