Position

Are you a Registered Nurse wanting flexible hours to fit in with your busy family lives? We have an exciting opportunity for 2/3 experienced Band 5 Staff Nurses to work with the Evening District Nursing Service, 5.30/6pm - 11.30/12am. You can be flexible on the evenings that you work and will be part of a fabulous and supportive team based at Withington Community Hospital, covering Central Manchester.

Key Responsibilities
  • Gain competence in clinical skills to enable delivery of quality patient care within the service
  • Take an active role in contributing to developing nursing practice
  • Maintain effective communication internally and externally to the organisation
  • Take responsibility for assessing, planning, implementing and evaluating evidence-based care for patients
  • Provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence
  • Identify patients whose conditions become unstable and liaise with doctors and acute services appropriately
  • Network with other professionals to co‑ordinate a multi‑disciplinary approach to care provision
  • Be responsible for multi‑agency care documentation in line with current practice
  • Ensure that patients can make an informed choice about their current and future care needs, keeping care patient centred and maintaining confidentiality in line with Trust policy
  • Prioritise own workload and work autonomously when necessary
  • Provide support, guidance and supervision to junior staff
  • Assess the level of ongoing nursing needs for people who may need long‑term care
  • Liaise with the patients GP to provide sufficient information for ongoing primary care to be provided safely and efficiently
