The successful candidate will re-assess care plans, contribute to team activities, and work to prevent unnecessary hospital admissions while delivering compassionate, patient-centered services.

Qualifications

• Current driving license and access to a vehicle.
• Community nursing experience preferred.
• NHS banding and terms of service understood.

Responsibilities

• Contribute to holistic assessment, planning, delivery and evaluation of patient care.
• Implement, monitor and maintain high standards of care in the community.
• Re-assess and re-evaluate care programmes as required.
