Overview
The Ambulatory Wound Care service offers an opportunity for an enthusiastic, dynamic Staff Nurse to join our forward‑thinking, expanding community team. We are a highly innovative group dedicated to delivering safe, quality care to our patients.
Responsibilities
- Assess, deliver and evaluate nursing care for patients with a wide range of health needs and wound care in a clinical environment.
- Support the team in promoting the Trust’s values and behaviours.
- Work independently, manage time, organise tasks and maintain clinical excellence.
- Assist in developing and implementing personalised care plans and holistic nursing care for patients.
- Ensure effective communication with patients, carers and other health professionals and other agencies to guarantee appropriate service delivery.
- Record patient contacts accurately on EMIS Web and Trust documentation, maintaining confidentiality and adherence to NMC guidelines.
- Perform wound care, leg ulcer assessment and management, and other clinical tasks as required.
- Recognise, interpret and analyse clinical data related to patients' needs and make appropriate referrals when risks exceed scope of practice.
- Supervise and delegate aspects of nursing care to patients and carers where safe and appropriate.
- Maintain accurate personal and professional records and adhere to MCHFT policies, procedures and the Nursing & Midwifery Council (NMC) guidelines.
- Report accidents and incidents, and participate in investigations according to the Incident Reporting System.
- Contribute to quality measurement and monitoring of standards.
- Perform non‑medical prescribing in accordance with MCHFT formulary if qualified.
Qualifications & Attributes
- Band 5 Staff Nurse, newly qualified or with community experience; previous experience not mandatory.
- Strong clinical and digital skills, capable of supporting specialist teams such as TVN, podiatry and vascular services.
- Excellent communication, interpersonal and team‑working skills; adaptable, reliable and approachable.
- Ability to work independently, manage workload efficiently and operate within the service footprint (Central Cheshire Integrated Care Partnership).
- Open to working in other areas where service needs require.
- Commitment to patient education and shared care approach supported by digital technology.
Working Hours & Conditions
Hours: 09:00 – 17:00, five days a week. Some weekend and bank holiday cover will be required.
Driving: Must have access to a vehicle to cover the service as needed.
