A rare opportunity has arisen for a Community Specialist Practitioner within our friendly Community Integrated Care Team in Upper Lea Valley.
The Specialist Practitioner will work within the Integrated team of Community Nurses, Therapists, CPNs and Social Workers. You will be responsible for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life. This will lead to increased patient choice and prevent unnecessary admissions to the hospital and reduce the stay of necessary hospital admissions.
The post-holder will be an excellent communicator and work well both individually and within a team. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter‑professional team. An extensive experience of working with older people is required and evidence of continuing professional knowledge and development.
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a maximum payment of £1,845 per annum (pro rata where applicable).
