Job summary

Liquid Personnel is recruiting an experienced Social Worker for the Community Wellbeing and Social Work Team (CAAsT) in Herefordshire. You will manage a varied caseload of adults with needs across older people, physical disabilities, learning disabilities and mental health, delivering person-centred support.

You will carry out Care Act and Mental Capacity Assessments, safeguarding enquiries, CHC work, Section 117 duties, and contribute to DSTs and support planning, collaborating with carers,

Qualifications

• Professional qualification in social work and registration with Social Work England.
• Significant post-qualification experience in Adult Social Care, community based.
• Strong knowledge of Care Act 2014, MCA 2005, safeguarding and CHC frameworks.
• Experience completing Care Act and Mental Capacity Assessments and care planning.

Responsibilities

• Manage a caseload of adults and undertake assessments and interventions per statutory duties.
• Conduct Care Act Assessments and Mental Capacity Assessments.
• Carry out safeguarding enquiries including Section 42.
• Develop and review person-centred care and support plans.
• Undertake Continuing Healthcare (CHC) assessments and contribute to DSTs.
• Complete Section 117 aftercare processes.
• Collaborate with carers, families, health professionals and partner agencies.
• Prepare clear assessments, reports and case records.
• Participate in multi-disciplinary meetings and professional forums.
