Job description

Please note, at this stage, the role is open only to internal staff (including NHSP and Agency staff currently on placement within our organisation) and staff substantively employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Any individual who is at risk of redundancy will be given priority, provided they meet the essential criteria.

We are seeking a qualified nurse to come and work in the SWK MHT+ team. If you are interested in older adults as well as in working age adults and keen to develop your expertise in initial assessments and therapeutic interventions, this is the role for you!

Successful applicants will be working in our combined memory clinics alongside the multi-disciplinary team, including psychiatrists, psychologists, nurses, an operational team manager, clinical lead and administrators.

The service is based in Tunbridge Wells but operates clinics in Sevenoaks too, so please note there may be an expectation to travel between sites as part of this role.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job
  • Deliver assessments, in a combined clinic setting, as home visits and in care homes where appropriate.
  • Act as a role model and support new team members through demonstration and explanation of tasks and techniques.
  • Facilitate, support and assess staff undergoing personal and/or professional development.
  • Undertake and maintain all essential training for the role.
  • Recording and documenting interventions delivered in a timely manner as per professional and local standards using KMPT systems.
  • Coordinate and make recommendations to the care plan process.
  • Act as Practice Assessor.
  • Assess, act on and report any safeguarding concerns.
