About the Role:
We are seeking a dedicated and experienced RMN, CPN, or Social Worker to join our team at Newark and Sherwood LMHT. The successful candidate will be responsible for conducting initial assessments for patients referred to the Local Mental Health Team, documenting assessments, and formulating appropriate ongoing care plans. The role also involves making referrals, liaising with other agencies, and identifying safeguarding concerns to escalate as necessary.

Key Skills & Qualifications:
– Qualified Mental Health Nurse (RMN), Community Psychiatric Nurse (CPN), or Social Worker (SW)
– Experience in Adult Mental Health
– Strong assessment and formulation skills
