Job description
Role Overview

As part of the ongoing commitment for the Assertive Outreach Team based within the Community Mental Health Services, an exciting opportunity has arisen for the position of Community Psychiatric Nurse at Grays Hall, Thurrock, Grays, Essex for an initial fixed term contract of 12 months with the potential of an extension.

Responsibilities
  • Act as the Care Co‑ordinator within a Multi‑Disciplinary Team, managing coordination of care needs, planning and implementation of evidence‑based care for a defined group of service users and their carers within a defined catchment area/locality.
  • Provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Team Lead in their absence as appropriate.
  • Serve as a role model and resource for junior/less experienced staff.
  • Work under the overall supervision of the Team Leader but undertake duties of the post without direct supervision.
  • Carry responsibility for the assessment and advising on the management of patients presenting to the Community Mental Health Team.
  • Ensure ongoing engagement with patients with Severe and Enduring Mental Health Illness, including Dual Diagnosis, history of disengagement, non‑compliance with treatment and revolving door admissions.
  • Maintain comprehensive assessment skills in physical health care, health promotion, social inclusion and recovery.
  • Demonstrate competence in prioritising workload in a busy environment and a proven ability to problem‑solve and deal with everyday practice issues.
  • Work on a 365‑days a year rota basis, including weekend shifts.
Qualifications & Experience
  • Qualified Mental Health Practitioner with experience in community care.
  • Proven experience of undertaking comprehensive assessments and inter‑agency working.
  • Ability to work on own initiative within a Multi‑Disciplinary Team.
  • Strong organisational skills and the ability to prioritise workload in a busy environment.
  • Experience in dealing with patients with severe mental illness, dual diagnosis and history of disengagement is desirable.

Additional Information

As a newly appointed employee, you will be responsible for incurring the cost of your initial DBS check relevant to your post; the amount will be deducted from your first salary with the Trust.
