Job description
Community Physiotherapist (General Rehab and Falls)

To provide high quality effective Physiotherapy assessment and treatment/management programmes for adults presenting with rehab needs in the community in Haringey locality team; who require a domiciliary service or out-patient service. To use highly specialist Physiotherapy knowledge and skills to manage own complex caseload, as an autonomous practitioner.

To act as a source of expertise on Physiotherapy management of adults in the community; providing advice to patients, carers, all grades of Physiotherapists, other health care professionals, consultants, general practitioners and social services.

To support the use of evidence-based practice within the service, incorporating national guidelines e.g. NICE and NSF into practice.

The post holder will provide supervision and mentoring to junior Physiotherapists and to assistants and students where necessary. The post holder will Lead and Manage Blue badge Mobility assessments and work closely with haringey parking services.

Main duties of the job

CLINICAL

To deliver patient care based on principles of best practice, achievable through direct patient contact or via supervision.

To carry out specialist, comprehensive assessments of patients with a complex presentation, using investigative and analytical skills formulating individualised management and treatment plans.

To work effectively with clients, their carers and colleagues around individual case management; setting client centred goals and ensuring all parties are working together to achieve the goals.

To provide specialised Physiotherapy interventions at home or in a clinic setting as required, collaborating with colleagues in the community and secondary care health professionals to promote continuity of care.

To ensure a timely response to functional changes or relapses.

To provide specialist assessment for and provision of adaptive equipment such as specialist seating in line with national and local guidelines.

Person Specification
Qualifications
  • Degree or Diploma in Physiotherapy
  • Registration with the Health Professions Council
  • Higher degree relevant to the area of practice.
  • Member of the Chartered Society of Physiotherapy and relevant Clinical Interest Group.
Experience
  • At least 5 years post-registration experience, including extensive band 5 and Band 6rotations and evidence of dedicated clinical practice and Professional development in the area of physiotherapy.
  • Experience across a range of NHS sectors e.g. Primary, Secondary, Tertiary and Community Services.
  • Experience of organising, writing and delivering in-service training programmes
  • Experience of supervising Junior and non-qualified staff, including participation in personal development reviews.
  • Experience of clinical governance initiatives
Knowledge
  • Basic computer literacy, including Word and PowerPoint.
  • Knowledge of current NHS initiatives.
  • Understanding of Clinical Governance and its application to physiotherapy Services.
  • Knowledge of Physiotherapy professional issues.
  • Knowledge of a variety of physiotherapy concepts and Approaches and current best practice.
  • Knowledge of neurological conditions and pathologies and the Current medical management of such conditions.
  • Knowledge of other medical conditions which you may encounter including, but not exclusively, respiratory, orthopaedic and elderly care conditions.
  • Extensive knowledge of outcome measure tools.
  • Basic knowledge of the audit cycle and its uses.

  • Able to identify your own and staff training needs
  • Well developed counselling and advocacy skills to support staff, patients and carers.
  • Able to employ motivational and influencing skills with staff, patients, carers and other professionals.
Other Qualities
  • Willingness to work on any site as service requirements dictate.
