Job summary

We are seeking an enthusiastic and motivated Senior Physiotherapist to join our Integrated Prevention and Reablement Services within Wrexham Social Services. This varied and rewarding role offers the opportunity to make a real difference, supporting people to remain independent and live safely in their own homes.

You will work closely with hospital teams to support safe and timely discharges, while collaborating with social workers, occupational therapists, social care assessors and other health and social care professionals. You will undertake physiotherapy assessments in people's homes, taking a holistic approach to identify the support needed to meet individual goals and needs. This may include supporting discharge from hospital, responding to periods of crisis, or providing reablement interventions to help people regain independence.

A key aspect of the role is ensuring people receive the right support at the right time, with regular reviews to ensure changing needs continue to be met. You will also support and supervise junior physiotherapy staff, helping to develop their clinical skills and confidence.

This is an excellent opportunity for a physiotherapist who enjoys multidisciplinary working, values person-centred care and is passionate about promoting independence. You will join a service where partnership working is central and where your contribution can have a significant impact on individuals, families and the wider community.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
