Job description
Overview
  • Short term support for people who require assistance with activities of daily living
  • A rehabilitation service, including a range of therapies
  • Community rehab for people with long term conditions and frailty
  • In-reach team to assess and provide rehabilitation programmes to patients that are admitted to the spot purchase bed in Brighton and Hove - we cover weekends in the beds team
  • Discharge planning with access visits to facilitate patients to transfer the care from nursing homes to their own homes
Key Responsibilities
  • Calling all registered Physiotherapists looking to join a forward‑thinking team - you could be an essential part of one of our Therapy Hub teams in Sussex
  • Joint patient visits, liaison with staff at all levels, providing feedback and escalating concerns
  • Working with patients with a wide variety of pathologies and medical conditions and visiting patients in their own homes or in‑reach to nursing homes
  • Assess, treat and monitor patients with long term conditions and frailty, as well as providing rehabilitation within their own homes
  • Experience of working in teams alongside Band 3 and 4 unregistered staff and enjoys helping others to learn
  • Full UK driving licence and access to a car is essential due to the domiciliary nature of this post
  • Subject to successful interview, references, DBS clearance and Occupational Health checks

Employment Details

Full time 1.0 WTE. We will consider applications for any part time hours (at least 0.4 WTE)/job sharing or compressed hours
