Job summary

Zohorecruit is seeking an experienced Occupational Therapist for a 7 months contract based in Horsham, West Sussex. You will assess adults in community settings, create person-centred plans and deliver specialist interventions to promote independence.

The role involves working in customers' homes, with office days as part of a hybrid pattern. You will collaborate with multidisciplinary teams and maintain thorough records, ensuring quality care and measurable outcomes.

Qualifications

• Qualified Occupational Therapist with recent adult social care experience.
• Experience completing Occupational Therapy assessments.
• Experience in Reablement, Rehabilitation, Community Services, Hospital Discharge, Adult Social Care or Independence Services.
• Experience developing and reviewing care and support plans.
• Experience working with adults with a range of physical, cognitive and functional needs.
• Strong assessment, analytical and problem-solving skills.
• Experience working within multidisciplinary teams.
• Experience managing a caseload independently.
• Excellent report writing and record-keeping skills.
• Good IT and communication skills.

Responsibilities

• Undertake comprehensive Occupational Therapy assessments within the community.
• Support customers to maximise independence and remain safely in their own homes.
• Establish meaningful and achievable goals with service users.
• Deliver specialist Occupational Therapy interventions.
• Develop, implement, and review support and rehabilitation plans.
• Monitor progress and adapt interventions to achieve the best possible outcomes.
• Work closely with reablement providers and multidisciplinary teams.
• Provide professional advice and recommendations regarding independence and daily living activities.
• Maintain accurate case records and assessments.
• Apply evidence-based Occupational Therapy practice.
• Manage a varied caseload across the North and West areas of West Sussex.
• Work collaboratively with health and social care professionals.
• Promote person-centred practice and positive outcomes for service users.
