Job summary

Sirona care & health, England's largest provider of adult and children's community services, is seeking a Band 5 Occupational Therapist for our Integrated Neighbourhood Team in Bristol and surrounding areas. You will assess, plan and deliver home-based rehabilitation to support independence and safe discharge from hospital.

Qualifications

• Initial holistic assessment, implementation and evaluation of programmes of care for patients with complex needs.
• Use clinical reasoning to assess, plan, implement and evaluate patient-centred interventions.
• Collaborate with patients, carers and family to identify goals within the care plan.
• Provide guidance and supervise team members within occupational therapy competencies.
• Maintain accurate, legal records and participate in training as required.

Responsibilities

• Be responsible for the initial holistic assessment, implementation and evaluation of programmes of care for patients with complex needs.
• Use clinical reasoning to assess, plan, implement and evaluate patient-centred interventions.
• Collaborate with service users, carers and family to identify achievable goals as part of the care plan.
• Facilitate appropriate referrals to other services.
• Work within a multi-disciplinary team to share knowledge and provide specialist skills.
• Use occupational therapy skills in autonomous practice and arrange equipment as needed.
• Supervise peers, students and unregistered staff in OT competencies.
• Maintain accurate, informative records and participate in training.
