Job summary

HealthJobs Ltd is recruiting an Occupational Therapist for a community adults team in Crewe, Cheshire. The role involves home visits, assessing needs of adults with illnesses or disabilities, and arranging equipment and adaptations to support safe discharge from hospital.

You will liaise with housing, NHS, and voluntary organisations, ensure documentation is thorough, and support patients through the discharge process while maintaining HCPC registration and professional development.

Qualifications

• Diploma or Degree in Occupational Therapy required.
• HCPC registration required.
• Experience in physical disability Occupational Therapy preferred.
• Strong written and verbal communication skills are essential.

Responsibilities

• Assess needs of adults in the community.
• Determine equipment and adaptations required.
• Provide informed advice and guidance to service users.
• Complete assessments to support smooth hospital discharge.
• Arrange equipment to support safe discharge home.
• Liaise with architects on adaptation plans.
• Collaborate with housing, NHS, and voluntary services.
• Check adaptations meet assessed needs.
• Refer service users to rehabilitation services when needed.
