  • Lead the provision of cost effective, efficient and appropriate high quality patient care within the community setting.
  • Carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.
  • Line manage a nursing team, ensuring the professional and clinical development of team members.
  • Promote evidence based practice and act as an accountable role model.
  • Ensure, together with the team, actions are in accordance with the NMC Professional Code of Conduct and other relevant policies.

Overview

Within Aneurin Bevan University Health Board, our Clinical Futures Strategy sets out how we are moving to delivering care closer to home. Primary and Community Services are at the heart of the model and central to developing a new relationship with patients investing and strengthening community and social care services to support and treat patients in their homes and communities providing quality outcomes. In order to deliver the strategy 2 permanent positions have become available within the Bargoed and Pontllanfraith District Nursing Teams in Caerphilly locality.

This post has two opportunities:

  • 1 WTE permanent Pontllanfraith District Nursing Team
  • 1 WTE Permanent Bargoed District Nursing Team

We are looking to appoint someone who is passionate about providing high quality care and we are committed to developing and investing in our staff to improve patient outcomes. We want to ensure you have the opportunity to grow and progress whilst also supporting staff wellbeing.
