Central London Community Healthcare NHS Trust (CLCH) is seeking a Band 4 Nursing Associate for a 5-month fixed-term contract to support housebound patients in Barnet. You will work with district nursing teams to deliver safe, person-centred care in patients' homes, promoting independence and timely discharges.

Applicants should be registered Nursing Associate with NMC, hold a Foundation degree, GCSEs and Care Certificate, and have excellent communication and IT skills.

Qualifications

Registered Nursing Associate on the NMC register.
Foundation degree in nursing associate practice (Level 5).
GCSEs in Maths and English (minimum).
Care Certificate completed.
Experience in health/care setting or relevant healthcare environment.
Ability to work collaboratively in a multidisciplinary team.

Responsibilities

Provide nursing care to housebound patients in their home environment.
Deliver clinical activities and holistic care under delegation by a registered nurse.
Support discharge planning and reduce hospital admissions.
Utilise IT systems to support NHS processes and record keeping.
