Overview

Are you a passionate and experienced nurse ready to lead a dynamic team in delivering high‑quality, patient‑centred care in the community? We are seeking a motivated and visionary Team Leader for our North East District Nursing Team. This pivotal role combines expert clinical practice with strategic leadership. You will be responsible for the holistic assessment and treatment of complex patients, delivering evidence‑based care in diverse settings including patients' homes. You will lead efforts to reduce hospital admissions and facilitate timely discharges, promoting independence and wellbeing.

Responsibilities
  • Provide strong clinical leadership and management of the team.
  • Accountable for the management, development, and supervision of identified staff within the community team.
  • Overall accountability and responsibility for the named district nursing team.
  • Clinical holistic assessment and treatment of complex patients.
  • Work in partnership to reduce hospital admission and facilitate discharge.
  • Promote optimal levels of independence and wellbeing for patients.
