A healthcare provider in Coventry is seeking a Community Nurse Team Leader to deliver high-quality patient care in community settings. We have an opportunity for the position of Community Nurse Team Leader, working with our teams at Tile Hill and Willenhall. This post supports the Clinical Lead and Service Manager in the management and professional leadership of all nursing team members to deliver a high-quality service in compliance with statutory responsibilities, professional guidelines and the policies and procedures of UHCW NHS Trust. This post provides day to day professional and managerial input to the community nursing team within their area of responsibility.
The post holder is responsible for the delivery of the highest standards of patient centred care to patients and their relatives through the development and management of staff within their remit.
The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency, and experience and will lead and enable the team to provide a high-quality service.
A Full UK driving licence and access to a car that can be used for business use is essential. Please note an international driving licence is not accepted.
Secondment will be considered for internal candidates - If you are successful after interview, you must obtain approval from your current line manager to accept the offer.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.
The clinical responsibilities for this role as named nurse for a Community Nursing Team will include the assessment of patient need, delivery of care and evaluation of care interventions in line with evidence-based practice for patients who require expert nursing care at home.
