Key Responsibilities
- Continue the management of patients and/or staff within your clinical setting, working autonomously alongside your team lead.
- Take the chain of command in the absence of your team lead.
- Assess care needs and develop, implement, and evaluate programmes of care and holistic care packages.
- Set and maintain standards and supervise and deploy staff, as appropriate.
- Facilitate educational programmes for basic, post‑basic, and in‑service training in close liaison with the Learning and Development Team.
- Work closely with senior team members to facilitate patient‑focused, innovative, research‑based practice in line with Surrey Downs Health and Care objectives.
- Work as part of the team undertaking responsibilities negotiated and agreed with District Nurses.
- Treat all patients as individuals, respecting their privacy and dignity at all times.
- Assess individual care needs, implement and evaluate holistic programmes of care.
- Involve, support, inform and educate family/carers.
- Promote health and wellbeing of the patient.
- Develop effective communication and liaison systems.
- Maintain clear and comprehensive, signed and contemporaneous records according to SDHC procedures.
- Facilitate/undertake training and professional updating for the team and students.
- Maintain and monitor standards of evidence‑based nursing care.
- Maintain own professional and clinical integrity in line with NMC guidelines.
- Support/mentor team members and participate in clinical supervision activities.
- Work without direct supervision and liaise closely with the senior team member, deputising as required.
- Be responsible and maintain timely patient care.
- Maintain data entry and other record‑keeping duties.
- Work in cooperation and partnership with all other professionals and agencies involved in the care of the patient.
- Act upon procedures that protect vulnerable individuals when necessary.
- Participate in the SDHC Clinical Governance and Risk Management Strategies.
- Undertake any other duties consistent with the post.
- Staff may be reasonably requested to move base temporarily or permanently to meet service needs.
