Role Overview

We are looking for a caring, motivated, and dynamic community nurse to join our friendly, professional team. You will be part of the Stratton Community Nursing Team, based at Stratton Hospital, covering Bude and the surrounding area. We serve Stratton and Neetside GP practices within the local PCN.

Shift patterns are 0800‑1600 and 1400‑2200. This is a fixed‑term post until January 2027. The closing date for applications is 09 December 2025.

You will need to collaborate closely with GP’s, health and social care colleagues, including therapists and voluntary organisations, and demonstrate a commitment to team working and innovative delivery of excellent standards of care. An active enthusiasm for community nursing, autonomy in practice, and a desire for ongoing professional development are required. An understanding of the changing climate of care delivery in the community is also expected.

Main Duties

This is a community‑based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates must demonstrate how they will manage these travel requirements.

Following an initial induction and ongoing clinical supervision, you will be responsible for:

  • Assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, aiming for maximum independence and optimum quality of life.
  • Working in partnership with patients and carers to offer support and advice and promote informed choice.
  • Working autonomously to provide effective, evidence‑based care in line with competency and training records and organisational policies, procedures and guidelines.
  • Supporting all locality teams by working flexibly, moving to assist the team in most need on a daily, weekly or monthly basis as service provision demands.
