Job Summary

We are seeking an enthusiastic and compassionate Community Nurse / Practice Nurse to join our Health Inclusion Team (Integrated Neighbourhood Team). This rewarding role focuses on delivering high‑quality, person‑centred care to marginalised and underserved populations, including people who may experience homelessness, social exclusion, or barriers to accessing healthcare.

As part of our multi‑disciplinary team, you'll provide holistic nursing care in the community setting, primarily homeless hostels and patients' homes. Bring sound community/primary care nursing experience, including essential confidence in wound care management, alongside a passion for supporting vulnerable individuals to improve their health and wellbeing.

Key Responsibilities
  • Provide high‑quality, holistic nursing care within community and practice settings.
  • Undertake assessments, develop care plans, and review patient needs.
  • Deliver wound care assessment and management, ensuring evidence‑based practice.
  • Work collaboratively within the Health Inclusion Team (INT).
  • Promote self‑care and health education among patients.
  • Participate in outreach and engagement activities to improve access to care for marginalised groups.
