Overview

Cambridgeshire and Peterborough NHS Foundation Trust is seeking an enthusiastic, dynamic Band 5 Nurse to join our Intensive Support Team (IST) for adults with learning disabilities in Peterborough. This team offers a unique opportunity to utilise your experience and expertise in supporting adults with learning disabilities in crisis. The successful applicant must have NMC Registration as a Learning Disability Nurse (RNLD) or Mental Health Nurse (RNMH).

You will join a multi-disciplinary team and contribute to holistic care, with emphasis on Positive Behavioural Support (PBS). The IST supports adults with learning disabilities with a diverse range of complex needs, aiming to support service users to remain in the community during crises by offering additional mental health support, therapy, and treatment. We support transitions from out-of-area placements and work with services supporting young people to prevent them leaving Peterborough.

Responsibilities
  • The main role of a community nurse in IST is to triage and manage crisis referrals and participate in the delivery of specialist Learning Disability nursing under the guidance of a senior Nurse.
  • Participate in fortnightly multi-disciplinary team meetings and provide feedback on caseloads; attend other multi-agency meetings within CPFT and with key partners.
  • Collaborate with family, carers, Police, Probation, GP and other key stakeholders and partner agencies with appropriate information sharing.
  • Participate in writing Positive Behaviour Support (PBS) plans and risk formulations with other team members.
  • Travel around the county independently to meet strict time deadlines; hold a full UK driving license and have use of a vehicle.
  • Contribute to CTRs (Care Treatment Reviews) for service users in hospital, out of area settings, or in community settings as applicable.
  • Regularly monitor and review medication changes with psychiatry where required.
  • Ensure clinical practice is evidence-based and aligned with NICE guidelines and Trust policies and procedures.
Qualifications and Requirements
  • NMC Registration as a Learning Disability Nurse (RNLD) or Mental Health Nurse (RNMH).
  • Practical clinical skills with experience of supporting a person with learning disabilities who have behaviours of concern and communication needs.
  • Experience of working as part of a team and undertaking holistic health care; capable of accurate report writing; understanding legislation that guides practice.
  • Ability to work in a scientifically rigorous environment, with a PBS approach to care.
  • Willingness to travel independently around the county to meet deadlines; full UK driving licence and use of a vehicle.
