Sirona care & health CIC is recruiting a community nurse for Woodspring Integrated Neighbourhood Team. The post supports people with nursing needs in their own homes, offering holistic care and education within a multi-disciplinary team.

Applicants need an adult nursing degree and NMC registration. Full-time and part-time hours are considered, with in-team training and support provided, plus Agenda for Change pay and NHS benefits.

Qualifications

• BSc or equivalent in adult nursing.
• Evidence of continuing professional development (CPD) and lifelong learning.
• NMC registration and active practice in community health nursing.

Responsabilités

• Provide holistic and specialist nursing within a multi-disciplinary Integrated Neighbourhood Team.
• Support service users with nursing needs in their own homes.
• Tailor interventions to individual caseloads in a person-centred way.
• Help people manage health needs and promote self-care.
• Deliver education and health promotion to service users.
• Develop additional competencies in line with the team approach.
• Build skills across a range of nursing interventions through training and in-team support.
